Age Concern have a list of resources on their website for adapting homes for future needs.

Lifetime (or Universal) design is about making homes that people can live in and enjoy regardless of their age, mobility or stage of life. Accessible Properties provide Lifemark rated housing that is accessible to those with disabilities, they say it meets the long-term needs of their tenants and hasn’t increased building costs.
